Update: Both radios and the host firmware have been updated with the latest versions, but no effect on behavior.

I'm not the first person to work with this hardware, and it is possible that some (or any) of the device firmware is of mismatched versions. Can that cause this problem?

Yes, the switch was as indicated in the instructions. What I eventually did that worked was to change the driver and force it (against Windows's protest) to an older one (in particular, it was CDM v2.12.28 that worked).
